ISSUANCE OF EU DIGITAL COVID CERT & SAFEPASS TO PEOPLE VACCINATED/TESTED POSITIVE OUTSIDE THE EU, EEA OR SWITZERLAND [forms in English]

Issuance of the EU Digital COVID Certificate and SafePass to individuals who have been vaccinated or have tested positive for the virus in countries outside the EU, EEA or Switzerland

For the issuance of the EU Digital COVID Certificate (EUDCC) and SafePass for Cypriots and/or legal residents of the Republic who have been vaccinated or have tested positive for the virus in countries other than those of the European Union, the European Economic Area (Norway, Iceland and Liechtenstein) or Switzerland, the Ministry of Health reminds the following:

  • Citizens who have been vaccinated in third countries outside the EU, EEA or Switzerland and wish to obtain the EUDCC or SafePass should duly complete Form A and send it to the email address: eudcc.vaccination@moh.gov.cy, attaching all of the required documents mentioned in the form.
  • Citizens who have tested positive for the virus in in third countries outside the EU, EEA or Switzerland and wish to obtain the EUDCC or SafePass should duly complete Form B and send it to the email address: eudcc.recovery@moh.gov.cy, attaching all of the required documents mentioned in the form.

It is emphasized that in order to avoid delays and inconvenience, the applications must be sent fully completed with the submission of the required documents.

It is recalled that citizens who have been vaccinated or have tested positive for the virus in a member state of the European Union, in a state of the European Economic Area (Norway, Iceland and Liechtenstein) or in Switzerland, in order to obtain the EU Digital Certificate, they must contact the Authorities of the country where they have been vaccinated or where they have tested positive for the virus. In such a case, information regarding the procedure established in each country for the issuance of the EU Digital COVID Certificate is available on the website https://reopen.europa.eu.
