The Ministry of Health proceeds to vaccinate 16 and 17-year-olds for citizens who are not registered with the GHS.
As stated in a notice, for citizens who are of these ages and who are not registered with the GHS, the Application Form for Citizens who are not registered with the GHS should be submitted to the Ministry of Health.
The form must be submitted together with a copy of the ID card or other Identification Document and the Alien Registration Card, in the case of a foreigner.
Documents can be sent to the Ministry of Health, electronically at: covidvac@moh.gov.cy, by post to the Ministry of Health at Prodromos 1 and Chilon 17, 1449, Nicosia or delivered by hand to the Nursing Identification Department, Ministry of Health, Prodromos 1 and Chilon 17, Nicosia.
When they come to the Vaccination Centres, minors must provide the following evidence:
- Written Consent Form of Parents/Guardians for Vaccination of a Minor with Vaccine for COVID-19 signed by both parents/guardians, unless legally (with the submission of a court decision) this is not necessary. The Form should be delivered to the Vaccination Centres during vaccination.
- Identification document (identity, passport, Foreigner Registration Card, etc.) of the minor.
- Copy of an identification document from one of the parents/guardians. If the minor is accompanied by the parents/guardians (or one of them), they should present an identification document.
